Our Opportunity & Change project helps to support people with multiple and complex needs, and aims to provide long-term bespoke support to individuals to overcome these complex barriers.

We have dedicated Social Workers (one for Derby/Derbyshire and one for Nottingham/Nottinghamshire) who support our participants, as we recognise that many of those engaged within the programme have not had the confidence to access support or enablement services beforehand. Our Social Workers review the needs of each individual and look to see whether there might be any other external support or funding available to allow that individual to live independently and to ensure sustained independence. They also look to connect an individual to support locally to become more socially connected and less isolated.

An Advocate is essential throughout this in allowing participants of the programme to have a voice when they might not feel confident enough to assert their opinion or possess the knowledge of what support they are entitled to. An Advocate gains the individual’s consent to do so and will make the telephone calls and chase doctors’ appointments – to name just a couple of examples – which help to get an individual the support they are entitled to and will help them overcome multiple disadvantage.

Starting salary – £22,462.00
Salary scale – SCP 14-18
Contract – part-time, 21 hour post
Job Share – not suitable
Duration – to cover maternity leave ending no later than mid-June 2020
